This 40 year old came to me with a history of two prior breast augmentations. Her first surgery was nearly 20 years ago, above the muscle, type unknown. The second procedue was about 9 years ago and implants were changed to Allergan style 468 textured implants 350 cc placed above the muscle. As the years went on, the look deteriorated and demonstrated the small amount of native breast tissue she had. She wanted smaller areolas and a more natural look. We removed the implants through her old periareolar incisions, created a neo-sub-muscular space and redraped the skin with tailor tacking in a vertical mastopexy technique.
