This woman came to see me some time ago wishing a rather large breast augentation. She saw another surgeon who elected to go no further than 371 cc, but did perform a very good concentric mastopexy to improve her nipple areolar complex. When she returned to me, she felt that she was entirely too small. Her implant position was also asymmetric. We utilized sizers to determine how much more volume she wanted and then used the Vectra three dimensional photographic system to help her visualize what that volume could do for her. She accepted the fact that her nipple areolar complex levels would be uneven. We used an inframammary incision to create a biplanar subpectoral pocket. We employed Mentor smooth ultra high gel implants with a volume of 650 cc. She was pleased with the outcome of her breast augmentation.
