This patient presented to me with a history of a previous overly aggressive breast reduction, which was corrected with a submuscular placement of saline implants. She wanted a change. The size was not right. She was hyperdynamic. We also detected a volume difference. So, we entered through an inframammary incision, as we were uncertain how her reduction was performed. We found 240 cc saline implants filled to 275 cc. We also found total muscle coverage, which was the cause of her excessive implant movement. She wanted to be about 200 cc bigger, and her right breast was about 50 cc bigger than her left breast. After transforming the pocket to a biplanar pocket, we placed Mentor high-profile gel implants, 375 cc on the right and 425 cc on the left. We also lifted the nipple-areolar complex and altered the contour of her breasts with a vertical mastopexy. Her postoperative course was uneventful, and she was pleased.
