This patient came to me with an interesting history and specific desires. Previously, she had saline breast implants placed subglandularly with a concentric mastopexy. Her nipples pointed down and she felt that her breasts were too big. I felt that her breasts looked so big because of the fullness in her lateral breasts. So I recommended liposuction of the later breasts and axillae. The best solution for her breast shape was a vertical mastopexy. We considered simply removing the implants but I was concerned that they would look too small. So with all these considerations in mind, we planned her surgery and performed a removal of 150c implants filled to 170cc, replacement with Allergan shaped implants style 410 at 140cc in the subglandular plane, verticle mastopexy and liposuction of the lateral breast and axillae.
