This woman came to me wanting to change the appearance of her breasts. She had two prior augmentations and nearly no documentation. One procedure was above the muscle, the other below. Three dimentional sizing was performed to measure the staring volumes of her breasts. They were off by quite a bit. We came up with a plan to switch to anatomic cohesive gels and perform vertical mastopexy. Intraoperatively, I found that the implant was totally below the muscle on the right, above on the left. The breasts had no residual shape left. We placed the implants above what fragments of muscle she had, and performed a vertical mastopexy.
