This patient felt that her eyes made her look older than she wanted. She wanted to look like herself, only better. Examining her closely, her upper eyelids showed excess skin and fat in the nasal and central pads. Her lower eyelids showed fullness with a low lid cheek junction and a prominent tear trough. We removed excess fat and skin from the upper eyelids. In the lower eyelids, we performed a fat transposition to fill the tear trough and elevated the orbicularis muscle through an obicularoplasty The lower eyelids required minimal skin removal. As she healed, she looked like herself, only younger. She was pleased.
